How do I get started?

Who should not take a GLP-1 medication?
Anyone with a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia (MEN) syndrome, anyone with a history of bowel obstruction, and anyone allergic to the medication. These are not proven to be caused by GLP-1 therapy in people, but a small share of laboratory animals developed them, so Dr. Patel does not prescribe for patients at risk. He screens for each of these at the first visit.
What labs do I need before and during treatment?
Screening labs before you start: hemoglobin A1c, a lipid panel, thyroid (TSH and free T4), and a comprehensive metabolic panel for kidney and liver function. If your own doctor ran these within the last six months, Dr. Patel can use them. If not, the practice works with Quest Diagnostics. Monitoring labs repeat about every six months while you are on treatment.
What side effects should I expect?
The main ones are nausea, stomach cramps, and occasionally vomiting, and most people find them mild. They sometimes show up a couple of weeks in rather than on day one. Dr. Patel can prescribe an anti-nausea medication, and he will ask about anything else you notice to decide whether to continue, hold, or stop. Rarely, an injection can cause a local skin reaction, and, as with any medication, a serious allergic reaction is possible but very uncommon. Severe vomiting, fever, or new pain is a reason to call, not wait.
How much weight do patients typically lose?
On average, seven to ten pounds a month while the medication is working and the rest of the plan is in place. Individual results vary, and the pace slows as you approach your goal.
How do I keep from losing muscle?
Raise your protein intake, keep strength training every week, and take vitamin D, B12, and collagen. Dr. Patel goes over this at the first visit because rapid weight loss without it can cost muscle and leave skin loose.
Is GLP-1 therapy only for weight loss?
No. Dr. Patel also uses it for fatty liver disease (MASH or NASH), which most people with high cholesterol or prediabetes already have and which can progress to cirrhosis, and for prediabetes, type 2 diabetes, sleep apnea, and certain addiction disorders. The indication is decided at the consultation, and some patients do not need medication at all.
